Starting physical therapy at home should feel simple, clear, and supportive. At HomeCare Rehabilitation & Therapy, we guide patients and caregivers through every step of the process.

Who Can Receive Services
You may be appropriate for in-home outpatient physical therapy if you:
Have difficulty traveling to a clinic
Are recovering from surgery or hospitalization
Have balance, mobility, or strength concerns
Prefer therapy in a familiar home environment
Have Medicare or Ohio Medicaid coverage

How To Get Started
Step 1: Referral or Initial Call
A referral may be sent by your physician or healthcare provider, or you may contact us directly to inquire about services.

Step 2: Insurance Verification
We verify insurance coverage and benefits prior to scheduling services. Our team will explain any requirements and answer questions before care begins.
Step 3: Initial Evaluation
A licensed physical therapist will complete an in-home evaluation to assess mobility, strength, balance, and functional needs. A personalized plan of care will be developed based on clinical findings and patient goals.
Step 4: Ongoing Therapy Visits
Therapy sessions are provided one-on-one in your home. Progress is monitored regularly, and treatment plans are adjusted as needed.
