HomeCare Rehabilitation & Therapy provides outpatient physical therapy services in the home for individuals who prefer or require treatment in their own environment. All services are delivered by licensed physical therapists and follow federal and state guidelines.

Physical Therapy Services We Provide
Mobility & Functional Movement Training
We help patients improve their ability to safely move within their home and community. Treatment may include gait training, transfer training, and functional mobility tasks tailored to daily routines.
Strength, Balance & Fall Prevention
Our therapists design individualized programs to improve strength, balance, and coordination to reduce fall risk and promote confidence with movement.
Post-Surgical & Post-Hospital Rehabilitation
We provide physical therapy following orthopedic surgery, hospitalization, or skilled nursing discharge to support continued recovery and prevent functional decline.
Pain Management & Movement Re-Education
Therapy interventions may address pain, stiffness, and movement limitations through therapeutic exercise, neuromuscular re-education, and patient education.
Chronic Condition Management
We work with individuals managing conditions such as arthritis, neurological conditions, and general deconditioning to improve endurance, mobility, and quality of life.
