HomeCare Rehabilitation & Therapy accepts referrals from physicians, hospitals, discharge planners, case managers, and other healthcare providers. We also welcome referrals from patients and caregivers seeking outpatient physical therapy services delivered in the home.

All referrals are handled in a timely manner and coordinated directly with the referring provider when applicable.

How to Refer a Patient

Referral information may be submitted electronically. A member of our team will follow up to obtain any additional documentation needed to begin services.

What We Need to Get Started

To begin services, the following may be required:

  • Referral or order for physical therapy (when applicable)

  • Demographic and insurance information

  • Relevant clinical documentation

Our team will assist in obtaining any missing information.

What Happens After a Referral

Once a referral is received, our team will:

  1. Verify insurance eligibility and benefits

  2. Contact the patient or caregiver to discuss services

  3. Schedule an initial physical therapy evaluation

  4. Communicate with the referring provider as appropriate
